I hereby authorize East Cooper Meals on Wheels to perform a background check using the information I provide on the SC Law Enforcement Division Criminal Records Check form. To you, our volunteer: Please do not provide money for the background check. ECMOW will do this on your behalf. Money from you is NOT required for this background check.

I hereby agree to grant to East Cooper Meals on Wheels, and its authorized representatives, permission to record pictures or video of my participation. I further agree that any or all of the material may be used, in any form, as part of any future publications, brochure, or other printed/online materials used to promote East Cooper Meals on Wheels, and further that such use shall be without payment of fees, royalties, special credit or other compensation.
